Recorded Webinar: An Interprofessional Approach to Treating Chronic Kidney Disease (CKD)

According to the National Kidney Foundation, one in three US adults is at risk of developing chronic kidney disease (CKD). Factors contributing to CKD include diabetes, hypertension, smoking, and obesity. The interdisciplinary care team – which can include other physicians, advanced practice providers, nurses, dietitians, pharmacists, and social workers – has emerged as one solution to improving CKD care. Studies suggest that interprofessional care teams may improve patient education and preparedness prior to kidney failure, both of which have been associated with improved health outcomes.
Join Victor Yu, PhD, RD, BC-ADM, and Azeem A. Mohammed, MD, for an interprofessional webinar focused on the collaboration efforts of the care team when treating people with kidney disease. In this in-depth look at CKD, Drs. Yu and Mohammed will provide an overview of the pathophysiology of the kidneys; explain nutrient needs and the importance of nutrition assessments; and discuss not only the roles of the care team members but also how collaboration among the team can result in improved patient health care outcomes and understanding of living with and managing CKD. Participants will engage with health care peers in breakout sessions to discuss strategies and tools to help create, implement, and improve interdisciplinary collaboration.
After completing this continuing education course, health care professionals should be better able to:
- Describe the pathophysiology of the kidneys and the effects medical nutrition therapy have on improving the quality of life for those who suffer from CKD.
- Explain the nutrient needs and relay the importance of using nutrition assessments for people with CKD.
- Discuss the roles of and implement strategies to have meaningful interaction with various care team members when treating patients with CKD.
Dr. Azeem Mohammed was born in India and is a graduate of the Deccan College of Medical Sciences. He finished his residency in internal medicine at St Barnabas Hospital, affiliated with the Albert Einstein College of Medicine in New York prior to completing his fellowship in nephrology at Augusta University. After four years as faculty in the Division of Nephrology, he moved to Atlanta as a staff nephrologist at the Northside Hospital System.
Dr. Mohammed has over six years’ experience in nephrology, with special interest in acid-based disorders, acute renal failure, and continuous renal replacement therapies. In addition to being an exemplary teacher, he is well-published and served as an investigator on multiple clinical trials in the field of acute and chronic dialysis.
Dr. Mohammed is board certified in both internal medicine and nephrology by the American Board of Internal Medicine. Dr. Mohammed resides in Atlanta, GA, with his wife and two sons.
Victor Yu, PhD, RD, BC-ADM, has over 30 years of experience in health care, nutritional services, diabetes education, and teaching in both the Army and civilian environments. He is an active practicing clinician and an experienced professor. He is employed full time as a renal dietitian at North Augusta Dialysis clinic, and part time as a professor of nutrition at the Georgia Military College. Dr. Yu practices what he teaches: he includes at least 150 minutes of physical activity in his weekly routine and follows a healthy diet.
